MDS Registered Nurse Coordinator

We are seeking an experienced Lead RN MDS Coordinator to oversee the development and completion of the Resident Assessment Instrument (RAI/MDS) in compliance with state and federal regulations and facility policies. This role is a key member of the interdisciplinary team, supporting quality of care, continuity of services, and individualized, patient-centered care planning.

Requirements

Current, active RN license in New Jersey
Proven experience as an MDS Coordinator
Hands-on experience with MDS completion
Strong knowledge of Medicare and Medicaid regulations and benefit guidelines
Excellent organizational, communication, and analytical skills

Preferred Qualifications

Experience in a Skilled Nursing Facility (SNF)
Familiarity with interdisciplinary care planning and quality improvement initiatives
